In a previous study, we evaluated this method in healthy subjects, showing good reliability during different acute oxidative stress-induced conditions (5). On the basis of this experience, we performed a sodium 13C-octanoate breath test in 10 patients (eight male, two female, body mass index 26.17 2.40 [mean SD], range 22.05–29.01) with clinical and histologic profiles fully compatible with a diagnosis of NASH: AST 39.16 6.4 IU/L (normal range: 7–45), ALT 69.8 44.02 IU/L (normal range: 7–45) and in 20 healthy subjects matched for age, gender, and body mass index as controls. All patients gave informed consent to participate in the study. We excluded patients with a history of ethanol (25 g/day) or drug consumption and other conditions associated with fatty liver, such as diabetes, celiac disease, obesity, bariatric surgery, malnutrition, and positive serology for hepatitis C virus, metabolic, and other viral hepatitis (6). Breath samples were collected before and every 15 min for 2 h after the ingestion of 100 mg of water-dissolved octanoic acid-1-13C, sodium salt, 99% atom isoenrichment (Isotec, Miamisburg, OH). Results were expressed as cumulative percentage dose of 13CO2 recovered at 120 min. 13 CO2 enrichment in breath was analyzed by isotope ratio, mass spectrometry (Breath Mat, Finninganmat, Bremen-Germany).
